Team Applications for 2011 ONE Lowell World Cup

In order to be considered for the 2011 tournament, your team must first meet the requirements listed within the guidelines detailed in our team rules document.

  The 2011 Team Application must be completed by the Team President (or Manager or Coach), who will be responsible for coordinating all activities for their team throughout the ONE Lowell World Cup event.

  Each team member will also need to sign a medical waiver and audio / visual release form.

  Each team manager should also fill out a team roster form indicating all team members, contact informaiton, and team colors.
